Brookstone Manor Assisted Living is an 80 bed assisted living facility and Brookstone Manor Specialty Care is an 18 bed secure dementia facility owned and operated by Partner Senior Living Options; that pride ourselves on our core values: professionalism, dignity, integrity, humor, and passion.

We are currently looking for care attendants (PCA/HHA/CNA)

Full time, Part time and casual positions available. Weekend rotation required.

Flexibility for Student schedules.

Responsibilities:

Assist residents with daily needs such as: Dressing, toileting, bathing, medication administration, feeding, assisting with transfers and ambulation, repositioning, brushing teeth.

Engage residents with daily activities such as: Bingo, group reading, exercises, arts and crafts, planting flowers, going on outings.

Prepare and serve meals and snacks to residents

Household chores such as: laundry, dusting, dishes, cleaning, vacuuming, and mopping.

Qualifications:

16 years of age or older

Compassionate and caring

Lift up to 50 pounds

Timely and task orientated

LPN
EXCITING, CHALLENGING and FUN! Full-Time LPN position in Grand Rapids, MN. This person must be willing to live and embrace our core values to include Dignity, Professionalism, Integrity, Passion, and Humor.

This LPN will work in coordination with the LPN Supervisor, RN and Executive Director within a dynamic setting supporting folks in an Assisted Living. This position will be responsible for both general nursing duties as well as staff support in supervision and training. Communication with the internal management team, residents, staff, physicians and families in regards to the health and well-being of the residents needs to be accurate and ongoing.

Some of the duties include but are not limited to: overseeing the administration of medications and treatments; maintaining accurate records; providing support, assistance, direction and supervision to HHAs; medication and medical supply ordering and monitoring and following through with physician orders. The LPN is also responsible for complete and accurate charting, assistance with resident admissions, and participating in staff education opportunities.

This position will generally work a day or evening shift-based on chosen block, but also be flexible based on the needs of building to include a fair on-call rotation which would likely include every other weekend.

Experience working with geriatric residents and persons experiencing various forms of mental health symptoms is preferred.

Candidates must pass the following: Back ground study with a non-disqualifying clearance, a pre-employment drug screen with ongoing random drug screening.
